When should this discussion happen?

Performance Management is an ongoing, continuous cycle of communicating and clarifying job responsibilities, priorities, performance expectations, and development planning that optimize an individual's performance and aligns with organizational strategic goals.
At minimum, an annual check-in should occur where the previous year is reviewed and the upcoming year and goals are outlined.

What form do I use?

The template below should be provided to the employee in advance of your meeting. Encourage the employee to complete the form and return to the supervisor prior to the meeting.
